    they are crazy and my neighbors run away……… i saw an article in the paper about someone who feeds homeless folks, but they gave no number or anything to donate stuff…………duh!

    and we are gonna have tomatoes out the ying yang in about two weeks. dh wants to can spaghetti sauce, but neither of us have ever done it. Only the quick canning with pickles…….

    It’s been a while since my last post, so I thought I’d drop by and wish you all a good week 🙂 I just finished reading everyone’s comments, and my thoughts are with those of you who are going through health and work-related challenges. I hope things start looking up for you soon ♥

    Hi everyone!

    My kiddo’s surgery went very well this morning. Thank you for thinking of her, Gaga! We were home by early afternoon. Her face is a little swollen and her speech is a bit garbled from the swelling in her throat, but overall she is doing well. She is drinking as much as she can and sleeping off the anesthesia. That’s the part she hates the most. It makes her really nauseated. She’s sleeping now, which means I finally get to rest! 🙂

    Miracle – I’m glad you enjoyed the honeycomb items! I sent you a couple more. As far as I know, that’s all the tradeable items in that theme. And yes, I posted the link to Lucas the Spider! The videos are the cutest things I’ve ever seen! My favorite episode is “Encore”. The look on Lucas’ face when he loses his harp is so precious. My husband loves “Giant”. He watches that 30 second video almost every day, and when Lucas closes his eyes and sings, “Ooohh”, my husband cracks up every time. He made Lucas the background for his computer. LOL. As far as Candy Bash goes, no trick to it. Just push through it. It’s awful! I just finished my challenge a few days ago because I hate the game so much. I played it a little here and a little there until it was finally done and was so glad when it was over!

    Catmom – Another awesome walk! You’re really getting around! Did you carry back all the items you bought?

    Cheese – Hugs, my friend! Congrats to you and Cleverlee on winning the Jumbleberry Blast trophy! That is a really hard one! I’m so glad my tutorial was helpful!

    Gaga – I’m sorry to hear that your workplace has become so intolerable. I’ve pretty well run the gamut of healthcare. Everything from nonprofit public community health and working in the ICU at a level 3 pediatric trauma center to managing a corporate health care office and case management for a Fortune 500 corporate insurance company. While I have tended, overall, to feel happier with my role in nonprofit institutions, I’ve enjoyed the corporate experience more in terms of employee satisfaction.

    Hospitals have changed SO much even since I entered the field 20 years ago. Most of them, at least in the Cincinnati area, were historically religious based. Doctors ran the hospitals. Now hospitals form alliances, and insurances companies and Medicare dominate healthcare decisions. It’s like a bad episode of Survivor. The whole system has become massively corporate. I’m so glad to be out of it, at least for the moment. If I have to go back, it will probably be to work for an insurance company, and even though I feel like I’m selling my soul to the devil, I enjoy the work and the atmosphere a lot more.

    I’m really sorry you’re so unhappy in your job, and I know you really want to ride it out until you retire. If I may offer a word of advice (which you may already fully realize)? In my experience, when there is change in management, whether it’s the managers themselves or the way things are done (or both), there is often a lot of staff turnover. And the bigger the changes, the more unhappy the employees are. A lot of times, it’s the most senior employees who have the most difficult time adjusting. I’ve seen this happen so many times. Longtime employees, really good ones, resist the changes brought about by new management, and many of them leave, either on their own or because management forces them out.

    The employees who can accept the changes and adapt are the ones who form the foundational core staff moving forward. I know that it is really difficult to embrace changes that you dislike and don’t agree with, especially if you’re holding on to the nostalgia of an older model. But new management will see their decisions as progress. It’s hard to invest yourself emotionally in a job like yours every day and simultaneously detach from the institution, but for the sake of your own psyche, it’s what you need to do. Your hospital might have once been a big loving family, but it’s falling victim to the same corporate changes that most healthcare organizations are facing. And to be fair, some of the changes are necessary because hospitals have limited control over their own fate now.

    I hope I’m not out of line in suggesting this, and I apologize if I am. But I’ve had this discussion with quite a few nurses, some of them my own employees. The more you can try to understand the rationales behind the decisions that are made, and support them, even try to embrace the changes that you can, the easier it will be for you to move forward without hating your job. If for no other reason, for your own mental state and self preservation until you can retire. There have been many things that I have disagreed with management on, but a lot of times I can at least accept it, maybe even get behind it, if I understand the reasons and the reasons are logical. And this can be really difficult for nurses.

    Most of us feel a strong sense of compassion for our patients and fellow employees, and that empathy and emotional attachment to them can cause our ethics to become too idealistic in this corporate healthcare arena. I really came to appreciate this when I worked in management. If it had been up to me from a strictly clinical point of view, I would have taken every patient who needed home health. I would have given them supplies if they couldn’t afford them. I would have paid my nurses and therapists more to make their visits, but that’s not sustainable for any institution. I had to turn away patients because their insurance wouldn’t cover their care. I had to let nurses go when our census dropped so low that we could no longer afford to keep them. They are tough decisions to make, but necessary to continue operating.

    I’ll tell you about one of the most difficult decisions I had to make as a nurse manager in home health. I took a referral for an elderly patient after she came home from the hospital. She had a managed Medicare plan. Her plan covered our visits initially to teach her or someone in her family how to do a procedure, which she would need to have done on an ongoing basis. She was absolutely physically incapable of performing the procedure herself, and she had nobody that was willing or able to do it for her on a regular basis. We informed her insurance company that she had nobody to do this procedure for her and she could not do it herself, because Medicare only covers the education, they will not cover a nurse going in weekly to do the procedure for the patient. However, from a clinical perspective, I believed that having a home health nurse go in each week to perform the procedure for the patient, which would be far more likely to keep the patient out of the hospital, was clinically beneficial to the patient, but also cost effective for the insurance company.

    Because this was a corporate, managed Medicare plan, the insurance company had the ability to authorize the visits based on medical necessity even if the procedure wasn’t covered by traditional Medicare. And here’s the kicker! I spent SO many hours working on this case with the patient’s PCP and her clinical nurse case manager at the insurance company. It got escalated to a physician at the insurance company and utilization review, and EVERYONE agreed clinically that a home health visit once a week was medically recommended for this patient. Verbally, I was told repeatedly by her insurance company to keep seeing her, but when I asked them for written authorization, they would not send it. So I had no proof of authorization, which is a huge liability. If we were to get audited (and insurance companies audit home health agencies, hospitals, and other healthcare organizations all the time), at best, they could make us pay back thousands of dollars for visits that we did not have authorization for, and at worst, we could be accused of Medicare fraud and end up with a Medicare audit, which is a million times worse than a JCAHO inspection.

    I can’t even tell you how much I wrestled with this decision. And I talked to my managers and QA about it for many hours. Ultimately, after several months of seeing this patient, I had to make the decision to discharge her from our care. The patient was SO upset, completely distraught, having nobody to help her with a nursing procedure each week. I spent several hours on the phone with her trying to help her figure out another option. Her PCP and the clinicians at her insurance company were very upset about us discharging her. My nurse who had been seeing this patient every week was heartbroken and really angry about having to discharge her, and I low key think she continued to go do the visits for a while voluntarily on her own time without pay until the patient could make other arrangements. And I told all of them that I completely empathized with the situation. I told her insurance company’s clinical review team that all I needed was written authorization, the exact same thing they were giving me verbally over the phone, and we would love to continue seeing her, but they refused to provide that. And that seriously jeopardized the company financially and legally, and since I was the manager making the call, it potentially jeopardized my license as well.

    These are the really tough decisions that healthcare managers have to make sometimes. And they can be awful. In my opinion, a good manager will wrestle with decisions like these. I made many decisions that I knew my nurses didn’t agree with. I even had one that quit, and a couple others threatened to quit. The best I could do was try to explain my rationales so that at least they understood where I was coming from, but at the end of the day, they had to grapple with it and decide whether to move forward with decisions they didn’t like, or move on. I valued my nurses and respected each of them, most of whom had been with the agency long before I joined the team. But they were all replaceable if they couldn’t accept my leadership. An open dialogue between managers and staff can really help to bridge the gap in situations like the one you’re in, Gaga. I don’t know if management is willing to include you in these decisions, or at least explain their rationales for them, but if so, it may help.

    So I don’t know if this helps at all, but my suggestion would be to try to wrap your head around the decisions as much as possible, rationally, not emotionally. I’ve had long talks with my managers in the past about why they’ve done the things they’ve done so that I can at least understand where they’re coming from. If you can try to think through these changes from their perspective, you may be able to appreciate where they’re coming from and ally with them, and if you respect your managers and they respect you, that tends to create a more tolerable working relationship even if it’s not perfect or ideal. Just some of my thoughts having worked on both sides of management.

    Good Morning All !
    RubyCube… Happy to hear that your daughter is home .. and everything went well !!
    Having been with the same institution for over 40 years… I have seen many administrations come and go…I have been through many changes and have always been able to adapt… but when the nuns gave up ownership five years ago… things have steadily gone corporate….Do more with less.. and less.
    I guess not that I am older I am more resistant to change..The whole atmosphere is different.. People no longer smile and greet each other as they pass in the hallways…I have a chance on Saturdays when I volunteer.. to go from floor to floor and stop at each division…Many times .. no signs of any nurses..aides.. almost like a ghost town…kind of scary …Who taking care of the patients ???
    I have resolved myself… to do the best I can… take one day at a time… and if and when the time comes they tell me to go….. I will but not look for another nursing job…..I will look back at my career and take pride in my work and move on…………
    On a lighter note…. Can’t wait for Wacky !! My favorite .. Hope to win all the prizes… Have started practicing WackyER in hopes of getting a high enough score..

    gaga, sorry to hear of your workplace woes!! I agree healthcare is hard. We are always short staffed where I work and even our admin admits you have to be crazy to work there. They don’t really offer great incentive for you to turn your life over to them. I had a bad day yesterday because I was over stressed. Scary thing is I’m trying to get into nursing school in January. I know what kinda mess I’m getting into yet trying anyway lol. I’m worried I’m too old for this stuff haha.
    Dee, I would love some cukes. I don’t like to eat them but I do love them in a glass of water. It gets me to actually drink the stuff. I’m more of a coke person but I’m trying to get clean lol. Haven’t drank any in 2 days now. I would love to try canning too. Don’t know what to can. Nothing grows well in this Florida heat!!!
    I am so thankful for all the work moms and the others put into this blog. I know I don’t comment much but I do love that its a place I can go and connect with people.

    The other big event this month is the Super School Giveaway. Top row: Marching Band Jacket (just as I predicted! *pats self on back*), Marching Band Shoes, Silver Marching Band Horn. Middle row: Game Day Bench, Game Day Bleachers, KVA Confetti Cannon. Bottom row: three unnamed foods that will most likely be new Recess foods. There’s a Super School Gift Box that I presume will be a floaty clicky and have the Marching Band clothing in it, and you will probably have to click a bunch of them to complete your marching band, because Ganz never makes collecting all of anything easy! The furniture might be available as a log-in gift or from WKN.

    finally got a popsicle, they come at six minutes

    which is kind of stinky as it takes me seven minutes to get to the wackyer prize level

    finding them is gonna be the hardest part for me……you can get five a day tho

    i get bored sitting around for six minutes tho.
